Chapter 207 - 0195: The Ultimate Hand Speed
Chapter 207: Chapter 0195: The Ultimate Hand Speed
“Should I show you another video? Xiao Song, please play it for everyone!” Director Han said calmly.
Song Zimo went on stage, inserted a USB drive into the computer, and opened a video. The video wasn’t as clear as the previous one, but it was still watchable.
A doctor with gloves on, not wearing a surgical gown but only scrub clothes, appeared to be outdoors because the patient was lying on the ground and the doctor was kneeling beside an open emergency medical kit.
The doctor quickly disinfected the patient’s chest and laid down a layer of perforated cloth typically used for minor surgeries like wound cleanings or suturing.
Afterward, he made a cut between the patient’s left ribs, used a chest opener to expose the chest cavity, and blood gushed out. He used his left hand to stop the bleeding, while his right hand cut open the pericardium and switched to a needle holder with a thread to suture inside. The whole operation was carried out amidst a fountain of blood. In a short time, the surgery was completed, the bleeding had stopped, and the incision was finally closed.
This video seemed unremarkable from start to finish. With just over three minutes in duration, it didn’t show much.
Director Han stood up and walked to the podium: “Did many of you fail to understand what just happened? This was a surgery performed by Fujiwara Masao when he was a lecturer, which is equivalent to a chief physician.”
“This surgery was performed on the scene of an accident, the entire operation took three minutes and twenty-four seconds. The patient had a ruptured heart. The disinfection, laying of the sterile drapes, the opening of the chest, pericardium, suturing the rupture in the heart, and closing the incision were all completed by one person in three minutes and twenty-four seconds.”
“Think about it, is three minutes and twenty-four seconds enough for disinfection? For opening the chest? Amidst the blood, can you locate the rupture in the heart? How to suture the rupture in the heart when blood is gushing out like a fountain?”
“That was an accident scene with no auxiliary equipment, a completely manual surgery which relied solely on Fujiwara Masao’s perfected surgical skills. Now, consider if three minutes and twenty-four seconds are sufficient to suture a finger wound?”
“This operation has become a classic example of trauma emergency surgery, unsurpassed to this day. This video is repeatedly broadcasted in advanced trauma surgical training courses in the United States and Europe.”
A ruptured heart! Surprisingly, it was managed in just over three minutes. This required extraordinary speed, courage, and mental endurance. Typically, trauma of this nature results in on-site death, as waiting for rescue on the accident scene and then transport to the hospital are time-consuming. This time-frame often causes such patients to lose their chance of rescue. Only someone like Fujiwara Masao, a freak, would dare and be able to perform this type of surgery on the scene.
“This video showcases trauma emergency surgery to its extreme. Fujiwara Masao is presently a professor at the Tokyo University Hospital, the chief professor of Orthopedics and the Trauma Emergency Center.”
“He has trained many doctors, including a few genius surgeons, such as the younger generation Takahashi. You will meet Takahashi at the academic conference. Whether it’s the anterior or posterior approach for the spine, he generally takes half an hour and hardly loses any blood. The time he takes to repair the aorta in critical conditions is no more than two minutes, reaching the level of his teacher.”
“There’s little over a month left until the conference. Doctor Yang will lead you through rounds of training, from surgical operations, emergency procedures, to team collaboration. We hope that through this training, everyone will level up. Director Tan, would you explain this video to everyone again?”
Director Han returned to his seat to rest. At this point, anesthesiologists, operating room nurse managers, nurses, and ER doctors and nurses gradually came in. The next step was to discuss the topics for the academic conference.
Dean Xia also came in. He beckoned, and Old Han stepped outside. The two of them went to Old Han’s office to sit for a while.
Dean Xia happily said, “Xiao Yang’s staffing issue has been resolved. The Human Resources Department will assign someone to guide him through the paperwork for his appointment. We specifically requested a staffing position for him.”
In G City, getting a staffing position in big hospitals is extremely difficult. The files of Doctors would generally have to be circulated in the talent market for one to three years before possibly getting the position. For those with a Master’s degree, there’s basically no hope, as some hospitals have an indefinite waiting period for positions.
Currently, a staffing position is extremely valuable. This is an unreasonable issue left over from the old system, but there’s no solution. Holding a staffing position involves aspects of salary, status, and retirement pension. In short, the difference between having and not having a staffing position is huge.
Some hospitals claim to have abolished the staffing system, but it’s not thorough. Most people can’t get a staffing position, yet a small number of people can. These few people hold great power.
Someone like Yang Ping, who has a Master’s degree, wouldn’t normally have a chance to obtain a staffing position at Sanbo. Dean Xia and Director Han, in an effort to retain talent, had gone to great lengths to secure a staffing position especially for him.
“Yeah, if we don’t offer competitive benefits, how could we retain talent?”
“You need to secure this young man, Old Han. We can’t let others headhunt him. I’ve heard there’s some movement on that front from the First Affiliated Hospital.”
“Don’t worry, they can’t steal him away.”
“We must not be careless, keep in mind that Old Su is famous for his love of talent.”
Dean Xia reminded him: “Others may not know, but you and I know that Xiao Su is affiliated with the operating room. It’s just that Old Su gave us an alert, and we pretended not to know. I’m just afraid of this relationship–”
Old Han tapped gently on the table, seeming lost in thought: “I understand this kid, don’t worry! Besides, if the day comes when he must leave, he will, and neither you nor I can stop him. We can only be happy for him.”
Dean Xia said: “Old Su and I share a deep bond, so I believe he won’t make too much fuss either. The conditions we offer Xiao Yang are quite good now, Xiao Yang should be content, and Old Su should be pleased.”
“Old Xia, don’t worry too much about this. Looking at this missy from the Su Family, I reckon she’s an independent kid. She didn’t go to First Affiliated Hospital but chose our Sanbo instead. Old Su wasn’t bold enough to make it public and he secretly asked us to take care of her without letting her know. Under these circumstances, he wouldn’t dare to make a move, right?” Old Han reasoned.
Dean Xia nodded: “In any case, let’s stay vigilant and try our best to keep the talented ones here. We’re getting old and the future of Sanbo depends on them. Seeing these young people really excites me. These kids are going to surpass us.”
The two chatted for a while and then returned to the lecture hall, where Director Tan was explaining a video. Everyone was captivated. The video seemed to create a huge impact, revealing the real gap.
“This video, without any editing, is their regular rescue record, that is, their actual level of competence, and all the rescuers involved are mid-to-lower-level doctors.”
“Let me elaborate, the job titles of Japanese doctors are complicated. In university affiliated hospitals, they are generally divided into assistant professors, lecturers, associate professors, and professors, corresponding to our resident doctors, attending physicians, deputy chief physicians, and chief physicians. In our terms, all the rescuers involved in this video are attending physicians and resident doctors. None of the higher-ranking doctors were on stage.”
“This shows that their overall strength is very high. You’ve just seen part of the rescue. There was also a patient with serious spleen rupture, who was given surgery right on the ambulance at the scene. Except for one death on the spot due to brainstem damage, all the others were saved. Of course, this has to do with the severity of the injuries. But it also proves a point. Once these patients were in their hands, as long as they were salvageable, they were stabilized quickly, everything became controllable, and the success rate of the rescue was maximized.”
“Look, from the moment they admitted into the emergency room to being in the operating room, they completed all pre-operation preparations and full-body CT scans. The surgery started only eight minutes after admission. That’s an incredible pace, isn’t it? From the ER directly to the operating room without any inspection, I’m afraid we couldn’t even make it to the operating room in eight minutes?”
“Upon entering, the blood transfusion had began. On the operating table, everything was made clear with a very detailed surgical plan. The entire surgery only lasted an hour.”
“Every step, every coordination has been optimized to the max. The information communication in pre-hospital emergency, emergency center, CT room, operation room is very smooth. After admission to the hospital, everything is completed in the emergency center, within the same floor, the same area. In less than two minutes, the CT report and images are out. This really tests doctor’s ability to read and interpret films. The surgical plan also came out, very detailed and accurate, which also severely tests a doctor’s decision-making level.”
“We should not only learn from them, but also from the United States, Germany, the United Kingdom in the future. We should learn from anyone who is more advanced than us. If we work hard and take it seriously, we can definitely surpass them. But as Director Han just said, empty words won’t help. To paraphrase a popular saying nowadays, don’t dream of defeating the enemy with just a keyboard–”
“With their methods as a reference, as well as other hospitals’, and considering our own situation, we’ve improved the process of our trauma emergency center. Take a look–”
“Ah, another point, why can they transfuse blood so quickly? Their emergency medical services on the scene are equipped with blood supplies. On-site blood drawing, on-site results, on-site blood type determination, on-site blood transfusion. When the patient reached the hospital, they took another blood sample, determined blood type again, and implemented subsequent transfusion. If the helicopter is dispatched, not only will it transport the most severely injured, it will also bring back blood samples from all the wounded for blood cross-matching in the hospital in advance to prepare for subsequent transfusion. Their means of bleeding control on the scene are also very extensive. In short, when it comes to bleeding control and transfusion, they really save every possible second!”
After Director Tan finished his speech: “Does anyone have any comments, or questions about the video?”
Usually in meetings, someone would break the tension with a joke or two during these pauses. But today’s meeting was gloomy, even the usual jesters kept quiet.
No one spoke, no one asked questions, not even a chance to take a sip of water. Under the tremendous pressure, it felt like the air was suffocating.
Damn it! Fujiwara Masao, this freak of nature, hosting a meeting for you guys and concurrently live broadcasting it to us at Sanbo. You are so capable, so many people can’t surpass you, why don’t you with the Yanks or the Germans instead of live broadcasting to us?
Publicly, it’s called exchange, but in reality, it’s just showing-off and bullying!
Some have already started swearing in their hearts, feeling terrible!
